PPO / EPO / HMO
Every insurance company will have a different provider network for different plan. If you visit the provider in network, you will not only be charged less, but also may get quicker access for that provider.
After 2014, we do not have any PPO network individual and family plan under ACA act, so we only could serve EPO and HMO plan in Colorado now.
Explanation of Benifits
What happened after you visit a provider
Most of the case, after you have visited a provider, you will not receive the bill if you have insurance. Instead of that the provider will send you bill to your insurance company, and the insurance company will provide you a document called explanation of benefits to show what is your responsibility and what is theirs. After that you should only receive the medical bill from the provider for your responsible charge.
